A CARD MUST BE KEPT ON FILE FOR ALL SERVICES

When you book, you are agreeing to all of our studio policies, so please make sure you read them in their entirety. You will receive a contract via email after booking, be sure to fill it out sign it and send it back within 48 hours of booking or your appointment may be canceled.

A credit or debit card on file, is required to book your appointment. Appointments are NON TRANSFERABLE. You cannot transfer your CANCELLATION fee to another date or time.

IF YOU HAVE ANY PREVIOUS PERMANENT COSMETICS, we require you to email us clear photos of your tattoo work for approval PRIOR booking, as not all previous work can be covered or tattooed over. If you fail to send in pictures of previous work, and correction cannot be preformed. your card on file will be charged 30% of your total service.

CANCELLATION FEE IS 30% OF YOUR SERVICE BOOKED.

If you cancel or reschedule within two weeks of your appointment time, your booking fee will NOT be returned or your card will be charged. If consistent rescheduling occurs, you may be asked to pre-pay the full cost of the appointment.

If you are more than 15 minutes late for your appointment, your card will be charged a cancellation fee, and you will be asked to re-book as We will no be able to accommodate for late arrivals as this will put us behind for our other clients and their scheduled times.

A valid, physical ID is required at the time of your appointment. If you arrive without a physical ID, you will be asked to re-book and be charged a cancelation fee.

PMU TOUCH UPS

If your 8 week touch-up appointment is included with the cost of your initial appointment, it must be scheduled within 6-12 weeks If you reschedule your 8 week touch up and it falls outside of the allotted time frame, you may be required to pay a set up fee of $75

If you are in need of a "yearly touch up" but Libby did not do your previous work, your existing work will need to be approved. once approved you must book a NEW CLIENT APPT.

Chemotherapy (Must be after 1yr of receiving treatment)

Accutane users past or present (call for more info)

Using dermatologist prescribed skincares

If you have any skin disorders in area to be tattooed

taking ANY blood thinning medications

Any scarring or hyper pigmentation issues

Are on and have been on Antibiotics or Steriods within the 14 DAYS

lip blush service: If you are prone to cold sores, We ask that our clients consult with a physician prior to being tattooed.

If you have ANY MEDICAL CONDITIONS (any autoimmune diseases, diabetes, steroids, etc.) please inform the shop PRIOR to booking. A doctors note may be required. If you fail to inform us of any medical conditions and we are unable to tattoo you, a cancellation fee will be applied.
